Nurse Practitioner – Greenwood Clinic

Summary: Responsible for providing primary care services, including assessing and managing patients in the facility following established standards and policies. Requires knowledge of the behaviors, developmental stages, physical and emotional needs, life cycle changes, culture, and comfort and care appropriate to the population served. Populations the department serves is identified in the department’s scope of service.
Essential Job Functions
- Provides excellent service to our customers following the Sunflower Greenwood Clinic core values, Sunflower Greenwood Clinic customer service standards and behavioral expectations.
- Maintains confidentiality according to HIPAA guidelines at all times.
- Collaborates with a physician whose practice is compatible
- Implement a Quality Assurance/Quality Improvement Program
- It consists of a sample of charts that represent 10% or 20 charts, whichever is less, of patients seen by the APRN nurse each month
- Maintain a log of charts reviewed which includes the identifier for the patient’s chart, reviewer name, and date of review
- Meet face-to-face with a collaborating physician once per quarter for the purpose of quality assurance.
- Implement a Quality Assurance/Quality Improvement Program
- Provides Patient, Staff, and Customer Safety:
- Creates a work environment that promotes safety for patients, staff, and customers.
- Reports any safety concern or near miss on the appropriate report form.
- Maintains awareness of safety at all times.
- Patient Care Assessment
- Diagnoses patient’s condition based on exam, tests, and reports, consulting with physician as appropriate.
- Performs physical examinations using appropriate medical equipment, instruments and tests before testing is ordered.
- Orders or performs various tests and analyses to provide information on patient’s condition.
- Prescribes and/or administers medications and treatments as appropriate to assessed medical conditions; vaccinates patients to immunize against communicable disease.Performs procedures as necessitated by patient’s condition, including CPR, and including suturing.
- Instructs patient regarding diagnosis, medications and treatments.
- Refers patients to community resources or medical specialists as appropriate for counseling or specialized diagnosis or treatment.
- Triages telephone calls and provides consultation.
- Demonstrates knowledge of the principles of growth and development (over the life-span).
- Educates patient regarding health and illness prevention.
- Collaborates with physicians in managing acute and long-term medical needs of patients. Provides monitoring and continuity of care between physician visits. (M)
- Documentation
- Obtains medical history as appropriate and documents.
- Maintains documentation in accordance with Sunflower Clinic standards and policies and regulatory standards.
Other Job Functions
- Follows facility and Sunflower Greenwood Clinic established policies, procedures and objectives, continuous quality improvement objectives and safety, environmental and/or infection control standards.
- Participates in performance improvement teams, attends departmental meetings and committees as directed.
- Expands and maintains professional knowledge through continuing medical education. Completes all competencies as required by Sunflower Greenwood Clinic.
- In collaboration with administrator of facility, develops educational materials and programs as needed for patients and staff.
Working Conditions
- May involve exposure to communicable diseases and other conditions common with medical facilities.
- Possible exposure to potentially infectious materials that require following established protocols and precautionary measures.
- May be required to wear protective clothing such as gown, gloves, surgical mask and/or goggles.
- Frequent contact with physicians, staff, patients and the public.
- Occasionally exposed to disoriented or agitated patients who need to be calmed/restrained.
Minimum Qualifications/Requirements
Physical and Mental Requirements
- Must have the analytical ability to evaluate problem situations and come to appropriate conclusions.
- Must use best judgment and discretion.
- Must have the ability to manage several assignments simultaneously and prioritize appropriately, to cope successfully with diversity and complexity, to integrate seemingly unrelated data and to reduce complex notions to relatively simple terms.
- Must have the ability to communicate effectively with peers, staff members, patients and others and to document using appropriate vocabulary, grammar and word usage.
- Must have the ability to achieve outcomes with respect to the coordinating of patient care activities with others.
- Must have the ability to respond and react immediately with unimpeded auditory perception of verbal/vocal instructions and auditory auscultation such as codes/emergency situations.
- Must have the ability to respond and react immediately to visual observation and in emergencies.
- Must be sufficiently mobile to access patients on different sides of exam tables, reach patients at different heights and positions from ground to six feet above the ground.
- Must be able to sit, stand, stoop or bend for extended periods varying on patient schedules and needs.
- Must have normal range of hearing, vision, manual dexterity, and physical capabilities consistent with a standard physician practice.
- Must have the capacity to document in a clear and concise fashion and according to JCAHO standards.
- Must be able to physically apply up to 10 pounds of pressure to bleeding sites, or in performing CPR.
Education and Experience
- Current licensure as a registered nurse in Mississippi.
- Successful completion of nurse practitioner certification.
- Current Mississippi and federal DEA as appropriate.
Other Skills and Knowledge
- Knowledge of legal and ethical standards for the delivery of medical care.
- Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment.
- Knowledge of relevant drugs and non-pharmaceutical patient care aids and instruct patients in current usage.
- Ability to work both independently and in a team environment.
