Telehealth is available for established patients at Sunflower Rural Health Clinic.

Sunflower Rural Health Clinic established patients can call (662) 756-4024 to schedule a time to talk about their health with a doctor or nurse practitioner by Zoom.

Telehealth allows our patients to visit with their provider via internet connection to have a face to face visit without leaving their home.

Telehealth is used for a wide range of reasons:

General Guidelines for Preventing the Spread of COVID-19

  • Wash your hands often with soap and water for a minimum of 20 seconds

Sunflower Rural Health Clinic, “Sunflower Clinic”, is open 7 days a week from 8am until 12am for non-emergent psychological, medical, and social services. Sunflower Rural Health Clinic is staffed by Physicians, Certified Nurse Practitioners and Licensed Nursing Staff for your medical needs. Sunflower Rural Health Clinic is located off the main entrance into the drive of North Sunflower Medical Center.

Sunflower Rural Health Clinic is an Accountable Care Organization serving our patients as a Patient Centered Medical Home. Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to the Medicare patients they serve. Coordinated care helps ensure that patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors. When an ACO succeeds in both delivering high-quality care and spending health care dollars more wisely, it will share in the savings it achieves for the Medicare program.

The Patient-Centered Medical Home (PCMH) is a model of care in which patients are engaged in a direct relationship with a chosen provider who coordinates a cooperative team of healthcare professionals, takes collective responsibility for the comprehensive integrated care provided to the patient, and advocates and arranges appropriate care with other qualified providers and community resources as needed.
