What is Swing Bed?
Swing Bed is an inpatient rehabilitation program designed to provide services to patients needing continued skilled care or therapy services, once they are ready for hospital discharge. Swing Bed provides extra care to ensure functional restoration is achieved both physically and psychologically.
The involvement of the patient’s family and caregivers is encouraged, as it is an integral part of achieving the highest standard of care.

The Rehabilitation Process
To be admitted into the Swing Bed Unit patients must have the potential to improve their functional abilities, be medically stable, be motivated, and have sufficient mental alertness to participate in the program.
- Pre-Admission Screening – Patients can be admitted to the program from home, a hospital, or other types of facilities if they have had a 3-midnight stay in an acute setting or it has been within 30 days of an acute stay in a hospital. Every potential patient is screened by a case manager and physician to determine if his or her condition may benefit from swing bed rehab.
- Assessment – Upon admission, each patient receives a thorough physical assessment by a nurse. The results of this evaluation, along with the patient’s personal goals, are used to create an individual treatment plan.
- Treatment – Treatment may focus on ambulation, muscle strengthening, and coordination, use of adaptive equipment, independent living skills, cognitive and communicative skills, bowel and bladder management, and psychological adjustment.
- Education – Areas commonly addressed include joint protection, fall prevention, pain management, and other vital techniques. Family members are also educated on how to assist with their loved one’s care and access community resources and support groups.
- Follow-up – Prior to discharge, the rehabilitation team will meet with the patient and family to make sure that the necessary support system will be in place once the patient leaves and to resolve any potential issues. Patients receive follow-up phone calls after discharge to ensure that they are progressing with their recovery and to address any remaining needs.
