Summary:  Responsible for the planning, coordinating, providing and documenting care for terminally ill patients from day of admission through day of discharge.  Utilizing physicians’ orders and professional skills, the Hospice RN develops and implements a hospice plan that meets each patient’s specific needs and is in compliance with federal and state regulations, reimbursement guidelines as well as the organization’s policies and procedures.

  • Performs physical and psychosocial assessment on all hospice patients.
  • Ability to adequately assess and reassess pain.  Utilizes appropriate pain management techniques.  Educates the patient and family regarding pain management.
  • Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible.  Reassess pain and notifies the primary care physician when pain is not manageable on the current medications.
  • Develops a written plan of care for each patient.
  • Provides skilled nursing care.
  • Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.  Offers ongoing support and palliative care.
  • Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
  • Follows up with obtaining lab results and notifying physician of any significant results.
  • Performs patient care responsibilities considering needs specific to the standard of care for patient’s age and clinical condition.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical condition.
  • Follows the five medication rights and reduces the potential for medication errors.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
  • Initiates needed referrals to other disciplines and/or community resources.
  • Supervises Hospice Aides on an every other week basis.
  • Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, new problems, etc.

Minimum Qualifications:

  • Graduate of an accredited school of nursing.
  • Current Registered Nurse licensure in the state of Mississippi.
  • Experience as an RN commensurate with one of the following:
    • One year acute care experience within the last two years;
    • One year home health care experience within the last two years;
    • One year community health or public health experience within the last two years;
    • One year hospice experience within the last two years.
  • Current BCLS certification.
